Community Access Group

Adult day care programs are supportive services designed to meet the needs of adults with disabilities who have difficulty performing activities of daily living. These programs provide for adults' physical and emotional well-being through healthcare services and social and recreational activities, all in a structured, supportive environment.

Private Home Care
Another Chance Home Helpers

Our mission is to provide comfort to you or your loved ones. We value our personal and professional obligations and keep our promises to enhance your daily living activities. Another Chance Home Helpers (ACHH) is a subsidiary of Another Chance CSI — licensed, bonded, and insured.

ACHH offers customized home assistance services available for both long-term and short-term needs, regardless of age, disability, or illness. All in-home care providers are carefully screened with extensive background checks including motor vehicle history, social security validation, and reference checks.

Community Living Support
Program 03

Community Living Support (CLS)

Community Living Support is an offsite service where your loved one is visited or transported to integrate with the community. Daily or weekly activities are made accessible by experienced, professional staff who provide a safe and rewarding day.

Our staff consists of Certified Nurse Assistants, Personal Care Assistants, Companion Sitters, and Nurses — all certified in BLS CPR and Crisis Prevention Intervention, with regular state-required training updates.

Supported Employment
Job Coaching (SEI)

Supported employment refers to service provisions wherein people with disabilities — including intellectual disabilities, mental health, and traumatic brain injury — are assisted with obtaining and maintaining employment. Our SEI program provides individualized job coaching and placement to help participants build lasting careers.

Nursing Services
Program 05

Skilled Nursing Services

Skilled nursing care refers to care or treatment that only licensed nurses can perform. This type of care is provided in homes, assisted living communities, Life Plan Communities, nursing homes, and other certified locations. Our nursing team delivers compassionate, expert care to individuals with complex medical needs.

Georgia Waiver Programs

We are authorized providers for major Georgia Medicaid waiver programs, giving more people access to the care they need.

Georgia Pediatric Program — a Medicaid program for medically-qualified children under 21. Managed by the Georgia Department of Community Health (DCH).

CCSP Waiver

Georgia Medicaid's Community Care Services Program (Elderly & Disabled Waiver) — designed to help frail older adults remain in their homes as an alternative to nursing homes.

SOURCE Waiver

Service Options Using Resources in a Community Environment — links primary medical care with approved long-term health services at home for frail elderly and physically disabled individuals.
