ACHH offers customized home assistance services available for both long-term and short-term needs, regardless of age, disability, or illness. All in-home care providers are carefully screened with extensive background checks including motor vehicle history, social security validation, and reference checks.
Community Living Support is an offsite service where your loved one is visited or transported to integrate with the community. Daily or weekly activities are made accessible by experienced, professional staff who provide a safe and rewarding day.
Our staff consists of Certified Nurse Assistants, Personal Care Assistants, Companion Sitters, and Nurses — all certified in BLS CPR and Crisis Prevention Intervention, with regular state-required training updates.
Supported employment refers to service provisions wherein people with disabilities — including intellectual disabilities, mental health, and traumatic brain injury — are assisted with obtaining and maintaining employment. Our SEI program provides individualized job coaching and placement to help participants build lasting careers.
Skilled nursing care refers to care or treatment that only licensed nurses can perform. This type of care is provided in homes, assisted living communities, Life Plan Communities, nursing homes, and other certified locations. Our nursing team delivers compassionate, expert care to individuals with complex medical needs.
We provide a range of durable medical equipment and assistive devices to support individuals with cognitive, physical, and medical disabilities in maintaining independence and safety in their daily lives.
